By submitting this form, I decline to have my banking or credit card information used for automatic payment drafting. I understand that all fees and invoices — including tuition, costumes, late fees, recital fee, and other studio purchases — are due per the Quick Reference Guide. I take full responsibility for payment of all charges. A $25 late fee may be applied if my account is more than 10 days past due. Accepted payment methods include personal checks, credit cards, debit cards, bank draft, and cash. Checks should be payable to "MSDA." All payments may be made in the office or through the Parent Portal.
